Case Manager II

Bridgeways is seeking an experienced Case Manager II to join our dynamic and forward-thinking Housing Program team. You will be providing supportive clinical services to facilitate housing placement, maintenance and mental health stability within our Housing programs. Duties to include facilitating access to safe, secure and affordable housing, providing supportive services to individuals to help maintain housing, entering required data into the Homeless Management Information System (HMIS), conducting housing meetings in Bridgeways’ owned/managed housing, preparing data and reports to funders and attending community meetings on Bridgeways’ behalf.

Ideal candidate will have:
-Master’s degree in a mental health or human/social services related field
-Agency-Affiliated Counselor Registration from WA State Department of Health
-Knowledge of Medicaid, BHO, WAC, and managed care client requirements regarding clinical services and documentation. Knowledge of and demonstrated skills in the effective provision of behavioral health services in adult mental health.
-Knowledge of current behavioral health care practices.
-Knowledge of and demonstrated skills in the processes of CQI, quality assurance, and quality improvement.
-Knowledge of local, state, and federal housing guidelines, trends, and funding opportunities.
-Knowledge of Landlord/Tenant Law, Fair Housing Laws, Homeless Management Information Systems, Shelter Plus Care Guidelines, and community housing resources.
-Strong documentation skills
-Excellent customer service and communication skills
-Creative problem solving skills

Case Manager I

Bridgeways is seeking a Case Manager to join our dynamic and forward thinking Employment Services team. You will be assisting individuals with mental health diagnoses build independence in employment by collaborating with the individuals, their support systems, the Division of Vocational Rehabilitation (DVR), and our partnering mental health agencies. This is a position that invites creativity in helping individuals overcome mental health related barriers to employment. You will collaborate with individuals in identifying their unique employment goals, help design plans to overcome barriers, with the ultimate goal being self-sufficiency. Activities include creating resumes, building job skills, practicing coping skills that would be used on the job, helping friends and family understand how they can help, networking with potential employers, reducing stigma around working with individuals impacted by mental health symptoms, helping individuals and employers understand how to create reasonable accommodations for disabilities, and more. Services are provided in both office and community based settings in Snohomish and Skagit counties.

•Able to be creative, flexible, dependable, and consistently present a positive attitude, demeanor, and professional appearance.
•Skill and ability in establishing trusting, working relationships with persons living with disability, respecting individual rights and preferences.
•Ability and skill in using principles of sales and marketing when searching for community job opportunities for individuals.
•Skill and ability in establishing strong working relationships with community employers, DVR, Social Security, and other social service agencies.
•Able to work a flexible schedule to meet the needs of the individual.
•Able to work as an effective team member.
