An exciting announcement: Bridgeways has become the PATH (Projects for Assistance in Transition from Homelessness) Provider for Snohomish County! This program assists individuals in accessing housing, mental health services, substance abuse treatment, disability benefits, and other services to stabilize them and facilitate recovery. Persistent and consistent outreach, and providing services at the individual’s pace, are the important steps needed to engage people with serious mental illness who are homeless, and PATH allows us to do just that.
Who is Eligible?
To be PATH ‘eligible’ means that an individual has a serious mental illness, or serious mental illness and substance use disorder, and is experiencing homelessness or is at imminent risk of becoming homeless.
Our case managers will engage in outreach services to find those who are experiencing homelessness to provide referrals for:
- Primary healthcare
- Mental health services
- Job training
- Educational services
- Housing
We believe this expansion of services will enable us to further fulfill our mission and better serve those in need. Rollout of the program began October 1st our outreach team will begin going out in the field in the next couple of weeks. This program is in partnership and in part supported by North Sound Behavioral Health Administrative Service Organization (BH ASO). This funding is ultimately from the U.S. Department of Health and Human Services making it a federally funded program.
Although the program is invested in supporting any individual that is experiencing a mental health concern and homelessness, the program has a particular emphasis on providing support for people of color and veterans. We see this program as not just aligning with our mission, but with our current work around Diversity Equity and Inclusion and ultimately, our values.
